Rescue Groups Can Help! 

We suggest completing the short form below for all animals in your care. The microchip number will then be searchable at the National Database with limited contact information available for the owner. You can also print one form for each new pet owner.

New pet owners who submit an application from participating animal care groups will need to submit a payment of $19.95 to complete their pet's profile page but they will not have an annual fee applied to their account. 

RFID-USA displays contact information for all brands of pet microchips submitted including Home Again ®, AVID™ AKC CAR/EID™, Digital Angel®,  ResQ®, ALLFLEX®, Schering Plough™, 24 PET WATCH™, Lifechip®, Banfield®, Crystal Tag™, Datamars™ & Destron Fearing™ once submitted.

This will soon be required to print No Annual Fee Rescue Applications. If you have already added this link to your website, you can submit the necessary information below.
